To slow the spread of COVID-19, FPG is now offering Virtual Visits

When you call us to make an appointment we may offer you a Virtual Visit rather than an in-person visit in our office. In a Virtual Visit you can remain at home and speak directly to your provider by internet video on either a smartphone or a computer that has a video camera and microphone. Your provider or their staff will contact you prior to your Virtual Visit to walk you through how to "join the meeting". If you are using a smartphone, you can get ready in advance by installing the "ZOOM Cloud Meetings".

While we would prefer to be able to care for you in person, it is extremely important that we limit our contact to slow the spread of COVID-19.

January 3, 2022: Our West Office at 6801 W 20th Street in Greeley is CLOSED due to severe flooding.

While we repair our office, the following providers will be seeing patients at other locations or using virtual visits: Joanna Branum, Daniel Clang, Tamara Clang, Scott Haskins, Paul Lobitz, Matthew Martinez, Chad Maurer, Chima Nwizu, Michelle Paczosa, Kyle Waugh
