Family Practice is seeking RN Care Manager to join our Comprehensive Primary Care team focused on improving quality of care, reducing healthcare costs and increasing patient satisfaction. Please see our website at: www.fpgreeley.com.

Duties:

  1. Works as part of the care team (patient, physicians and MAs) in the development of care plans for high risk patients. Helps to manage and communicate as needed regarding patient care plans to other parts of the team.
  2. Coordinates patient care with external healthcare organizations including hospitals, specialists, physical therapist, pharmacists, extended care facilities and other groups as needed.
  3. Educates patients in regard to their high risk diagnoses and helps them to recognize and overcome behavioral obstacles in their lives to gain better health.
  4. Helps to maintain patient health information in the EHR, including medical history, results of testing, and medication management.

Knowledge, skills and abilities:

  1. Demonstrates excellent leadership, interpersonal, problem-solving and organizational skills.
  2. Ability to deal with a wide range of customers, provide a high level of customer service, and show awareness and respect for diversity.
  3. Familiar with motivational interviewing and self-management support.
  4. Computer literate, being able to gather and analyze data through EHR.
  5. Exhibit sound nursing skills and judgment.
  6. Ability to work autonomously, and work as part of the care team.
  7. Demonstrate good communication skills, both written and oral.

Education and experience:

  1. RN with bachelor’s or master’s degree. Colorado license.
  2. Experience in primary care and care management preferred.

Salary Range:  $55,000 - $80,000

The Care Coordinator role is an integral part of the CPC+ team having the goals of improving the overall health of patients, increasing patient satisfaction and lowering health care costs. The Care Coordinator will collaborate with the team in the identification of patient biopsychosocial healthcare needs. They will work to access appropriate resources, develop plans of care meaningful to the patient and work to assist in preventing avoidable hospital admissions and emergency room visits.

Duties:

  1. Works as part of the healthcare team in the provision of care for all FPG patients. Collaborates in the development of care plans, matching type and intensity of services to patient needs.
  2. Provides telephonic support to FPG patients for hospital follow up, ER visits and other care management goals.
  3. Coordinates patient care with external healthcare and other organizations including but not limited to hospitals, extended care facilities and other community groups as needed. Accesses appropriate resources collaboratively with patient and family.
  4. Assists the Care Managers in patient education regarding their diagnoses and helps patients to recognize and overcome behavioral obstacles in their lives to gain better health. Uses motivational interviewing techniques.
  5. Helps to maintain patient health information in the EHR, including medical history, results of testing, and medication management. Documents information in a timely manner.
  6. Provides patient interventions safely and competently as training and background allows.

Preferred education and experience:

  1. RN or LPN. Must have strong experience in a health care setting and/or experience in Case Management, bilingual preferred
  2. Experience in primary care clinic setting.

Pay Range:  $20.00 - $30.00 per hour

Part Time Cleaning & Janitorial Position

After hours cleaning opportunity at various FPG buildings. This will be an employee based Janitorial candidate, approx. 20-25 hrs/wk. Successful candidate will have excellent attention to detail, genuine interest in cleanliness and sanitization, willingness to work evenings and weekends if needed. Minor lifting will be required. Previous commercial cleaning experience a plus.

Busy family practice hiring for part time Reception  position.  25 hrs/week, noon - 5pm shift at our Central  location. Rotating Saturday morning included. Successful candidate will be high energy, have excellent customer service skills, good computer skills and be a team player.  Duties include scheduling appointments, checking in patients, collecting payments, answering phones, and patient customer service.   Part time benefit package available.

 

Salary Range:  $13.50/hr - $15.00/hr

Come join a great team! Full time Medical Receptionist opening at a busy Family Practice.  Primary duties include:

  • scheduling of appointments either on the telephone or in person
  • checking in patients, verifying insurance, collecting payments
  • answering a high volume of phone calls
  • providing excellent patient customer service

Successful candidate will have strong customer service skills, excellent communication skills, great multitasking ability, attention to detail and experience with electronic health record systems  Excellent benefit package available.

Salary Range:  $13.50/hr - $15.00/hr

Job Description: Full-time position / variable hours – days, evenings and rotating Saturday, along with an excellent compensation and benefits package.   Physician Assistant/Nurse Practitioner for busy family practice clinic possibly working at multiple locations.  Looking for experienced applicant to provide healthcare services including assessment, diagnosis, treatment, and planning under the supervision of a physician.  Coordinates orders for lab, x-ray, tests, referral to specialists, etc.

Requirements:   State certification/licensure and accreditation.  Unrestricted DEA licensure.  Eligible to maintain status with Medicare and Medicaid.   Minimum of two years’ experience required, experience in a Family Practice setting preferred.
